Assesses, plans implements, and evaluates speech, physical, occupational, and audiology therapy treatment for inpatients, outpatients, home care services, residential care, long-term care, and other contracts as per the health systems' policies and procedures, utilizing equipment, supplies, and procedures appropriate to the ages of the patients and residents served, including neonate, infant, pediatric, adolescent, adult, and geriatric patient populations for the subsequent functions.

Identifies appropriate candidate for rehab.

Establishes an effective plan of care using appropriate skills for evaluation and screening, review of medical records.

Communication with patient/resident, family and physician.

Writes clear, concise, legible documentation that includes appropriate information for care planning, object and subject info, functional goals, progress notes, and D/C summary.

Demonstrates knowledge of Medicare/Medicaid/Private Pay guidelines and includes appropriate supporting documents for reimbursement.

Physician orders, re-certs, pre-certs, and documentation are completed within period designated by policy and procedures.
